Reablement Services

What is reablement care?

Reablement care, often known as intermediate care, is a specialized form of short-term home care designed to facilitate independence after a hospital discharge, an illness, or an accident. This targeted care approach supports patients in regaining their autonomy through structured assistance at home.

Unlike general post-operative or post-surgical care, which may not specify an end date, reablement care is distinctly aimed at achieving independence within a predetermined time frame. It provides tailored support that helps individuals transition smoothly back to their daily routines, promoting a quick and effective recovery process.

Understanding Reablement Care: Regaining Independence at Home

Reablement care is designed to help you regain your independence through support provided in your own home by local carers. This service typically lasts up to six weeks, with most individuals improving within two weeks.

The care includes assistance with daily activities aimed at enhancing independence, such as mobility exercises and routine personal care.

A comprehensive, integrated care team is a fundamental part of reablement care. This team often includes a range of healthcare professionals such as homecare workers, nurses, occupational therapists, physiotherapists, social workers, and doctors, all coordinated to provide a holistic approach to your recovery. This multi-disciplinary team ensures that all aspects of your health and well-being are addressed, speeding your return to everyday activities.
